Achieve offers Attendant Care, Day Treatment and Training, Group Supported Employment, Habilitation and Respite services. For those interested in receiving services please contact the HCBS Coordinator.

Attendant Care

This service is provided in the home and provides assistance to the individual to attain or maintain safe, sanitary living conditions and/or maintain personal cleanliness.

Day Treatment and Training

We provide training, supervision, activities as well as promote skill development, self care, communication and social interaction.  Service is provided Monday through Friday, 8am to 3pm for adults, children in school may attend before, after school and during school breaks, and summer program is 4 hours/day.

Group Supported Employment (GSE)

Opportunities are provided for participants to learn job skills, job training and job attainment.  This service may be combined with Day Treatment & Training. Job Coaches provide supports to the individual throughout the day including in competitive job assignments.

Habilitation

A variety of interventions such as habilitation therapies, special developmental skills, behavior intervention and sensorimotor development, are designed to maximize individual functioning.  Training in activities related to routine household maintenance, as well as to meet personal, physical and social needs to enable or enhance their ability to perform self-help skills, physical mobility and adaptive living skills.

Respite

Short term relief to the primary caregiver providing supervision in or outside the home.
